Co-amoxiclav 1.2g IV 8 hourly

or 625mg oral 8 hourly depending on severity

Teicoplanin IV 12mg/Kg, maximum 800mg, 12 hourly for 3-5 doses then once daily if to continue after review

plus

Gentamicin IV (Extended Interval Dosing as per guideline) Use gentamicin calculator. Max 3 days then review.

or, depending on severity,

Clindamycin 150-300mg oral 6 hourly, increased if necessary up to 450mg 6 hourly.
