True penicillin allergy:

Discuss with Microbiology

ORAL

COTRIMOXAZOLE PO 960mg BD (eGFR >35ml/min)

Or

DOXYCYCLINE PO 200mg OD

Duration Depends on eventual diagnosis

If IV treatment is still needed after three days, convert GENTAMICIN to:

TEMOCILLIN IV 2g TDS
