If severe infection/sepsis:
Piperacillin-Tazobactam (Tazocin) 4.5g IV 8 hourly
HAP ≥5 days

Levofloxacin 500mg PO 12 hourly (consider safety issues)

Add Linezolid 600mg PO 12 hourly if MRSA suspected/confirmed
