Important: Therapy
Co-amoxiclav 1.2g IV
If weight >100kg give additional dose of Amoxicillin 1g IV
(i.e. total dose = 2g Amoxicillin and 200mg clavulanic acid)
Notes:
If BMI >35 continue Co-amoxiclav 625mg PO 8 hourly for 24hr post-op
Caesarean Section
Important: Therapy
Clindamycin 600mg IV AND Gentamicin IV (refer to Drug Dosing Table)
Weight >80kg: increase Clindamycin dose to 900mg IV
Weight >160kg: increase Clindamycin dose to 1200mg IV
Notes:
If BMI >35 continue Clindamycin 300mg PO 6 hourly for 24hr post-op
Important: Therapy
ADD Teicoplanin to above prophylaxis (refer to Drug Dosing Table)
Notes:
No additional oral antibiotic required for 24hr post-op

All antibiotic doses should be given pre-skin incision.
Ensure that appropriate antibiotic re-dosing if >1.5 litre blood loss as per Drug Dosing Table (after fluid replacement)
