Many are viral. Illness resolves over 4 days in 80% without antibiotics.

Important: Therapy

Oral Amoxicillin

Notes:

Avoid or delay antibiotics in children without systemic features. Antibacterial treatment may be started after 72 hours if no improvement and presence of systemic illness.

Consider earlier treatment if deterioration, if systemically unwell, if at high risk of serious complications (e.g. immunosupression, cystic fibrosis), if otorrhoea or in children under 2 years of age with bilateral otitis media.

Optimise analgesia.

 

If severe

Oral/IV Co-amoxiclav

In penicillin allergy

Oral Clarithromycin

Notes:
