Caesarean section wound infection

Antimicrobial prescribing: NHS Lothian
NHS Lothian
If patient has fever or evidence of sepsis send blood cultures (10mls in each bottle).

Antibiotic recommendation in mild infection

Recommended duration = IV + oral = 5 days

Recommended Antibiotic  Flucloxacillin 500mg every 6 hours orally
Penicillin allergy (if breast feeding) Erythromycin 500mg every 6 hours orally
Penicillin allergy (not breast feeding) Clarithromycin 500mg every 12 hours orally

 

Antibiotic recommendation in severe infection

Recommended Antibiotic

Flucloxacillin 2g every 6 hours IV

If evidence of severe sepsis

ADD

Gentamicin is on or after day 3 postpartum use the NHS Lothian AMT intranet calculator otherwise follow Sepsis in Obstetrics guideline

AND

Metronidazole 400mg every 8 hours orally (500mg every 8 hours IV if oral route not available)
Penicillin allergy

Clindamycin 600mg every 6 hours IV

If evidence of severe sepsis

ADD
