RESTORE2^TM mini is a condensed version of the full RESTORE2^TM tool. It is designed to support care and residential home staff who cannot measure vital signs to:

Recognise soft signs that could indicate deterioration
Use the SBARD tool to record and communicate soft signs and other information in a consistent structured format.

Before you start: Does the resident have NEW ONSET of any of the following?

Stroke (facial/arm weakness, speech problems)
Central chest pain
Heart attack/cardiac arrest

If yes, URGENT ACTION NEEDED! Is this resident for escalation to hospital? Check Anticipatory Care Plan.
