This form is used to refer exceptional prescribing requests to the Exceptional Prescribing Panel:
- drugs not SMC approved
- drugs SMC approved but not for the indication requested
- drugs SMC approved but with not part of a local pathway
The use of established medications off label should be discussed with the lead cluster pharmacist rather than referred to EPP.
