- Check address & GP details.
- Check school details if appropriate.
- Check social worker details if appropriate.
- Explain the local referral pathway to antenatal care.
- Give the young person written confirmation to show to their GP/midwife or, directly refer them to antenatal care if they prefer you to do this.
- Consider special needs in pregnancy referral as appropriate to local pathways.
- Is this referral to a midwife for young mums?
- Agree a plan to ensure you can check the young person has been referred to obstetrics (that is verifiable either through checking the hospital EPR that a referral has been made or contacting the primary care team).
- Check on hospital EPR patient was seen.
Advise that you will share information with an appropriate professional (GP, social worker) if you cannot verify that the young person has told an appropriate person about their pregnancy (GP, social worker, parent).
