Live Active is designed to help build confidence so that participants can make positive lifestyle changes. Support is personalised to meet individuals needs. One to one catch ups and supported activity sessions help keep individuals on track to achieve their goals.

Referral by HCP. Complete appropriate referral form and forward to relevant site 

• require a BP reading within 6/12 of referral
• Clinical responsibility – only responsible for information on the date the form was completed. May want to check clinical portal if unsure of patients history.
