Safety knowledge

CONTRA-INDICATIONS/INTERFERENCES– SITUATIONS WHERE ROCHE INFORM TEST NOT MOST APPROPRIATE TEST - BLOOD SAMPLE SHOULD BE SENT TO LABORATORY INSTEAD

 

  • Galactosaemia – Galactose >0.83 mmol/L
  • Lipaemic samples – Triglycerides >20.3 mmol/L
  • IV ascorbic acid – Ascorbic acid >0.17 mmol/L
  • Haematocrit<10% or > 65%
  • IV N-Acetylcysteine

 

Impaired Peripheral Circulation – severe dehydration due to

  • DKA – Diabetic Ketoacidosis
  • HHS – Hyperglycaemic Hyperosmolar Non-Ketotic Syndrome
  • Hypotension
  • Shock
  • Decompensated heart failure NYHA Class IV
  • Peripheral arterial occlusive disease

 

Unwashed hands – unwashed hands, food residue, hand creams etc can interfere

Meter knowledge

Touch screen – Use finger only

 

Scanner window – Hold meter 10-15 cm away from barcode

 

Strip Port – only approved test strips to be inserted, make sure no fluid enters device directly.

 

Reading range

  • Measurement range – 0.6 - 33.3 mmol/L
  • Critical range – Low - 3.9 mmol/L High - 15.0 mmol/L
  • Normal range – Low - 4.0 mmo/L High  - 10.1 mmol/L

 

Operator ID – DO NOT SHARE BARCODES

 

Patient ID – MUST enter correct patient CHI Number

 

Access of stored data – Review Results option, can filter by date performed or CHI number

Test strips

Code chip – performed by Point of Care Testing Team to add new strip lot data to all devices

 

Calibration – calibration information downloaded from code chip

 

Expiry Date

  • Do not use past expiry date
  • Can be used up to expiry date if stored correctly

 

Storage

  • 2-30⁰C
  • 10-90% humidity
  • Keep test strip container lid tightly closed
  • Close test strip container lid immediately after removal of test strip
  • Use test strip immediately

 

Handling and insertion of test strip

  • Wash and dry hands
  • Wear gloves
  • Do not touch either end of test strip
  • Insert into test strip port with lettering facing upward

 

Sample application – apply to front edge of yellow window of test strip keeping meter level

Internal/external QC

Must QC every 24 hours – QC lockout if not done, will not be able to perform patient testing.

 

Other times to QC

  • Start of new test strip container
  • Start of a new lot of test strips
  • If questionable results are repeatedly displayed
  • To check performance of the meter

 

If QC fails

  • repeat QC ensuring correct level of QC used
  • repeated failures contact Point of Care Testing Team

 

QC Expiry

  • Unopened bottles stored at 2-32⁰C until expiry date
  • Opened bottles – 3 month expiry from date opened (write date on bottles)

 

EQA

  • Pink solutions sent monthly, to be performed as Patient Test

Scan barcode provided as patient ID and return results to POCT Team by date stated

Hand washing

  • Wash and dry operator hands before wearing gloves
  • Wash and dry patient hands

 

Sites, Method, Sample

  • Single use lancing device
  • Side of finger – not forefinger or thumb
  • Touch front edge of test strip to blood drop
  • Insufficient sample application will lead to erroneous results
  • Can enter up to 3 comments – only one custom
  • Dispose of consumables in sharps bin and clinical waste as appropriate

Dock meter on base unit to transmit results to SCI-DB

Cleaning and storage of meter

Cleaning of meter

  • Switch off meter
  • Clean after each use using Clinitex wipes ensuring no liquid enters test strip port

 

Decontamination - Decontaminate using <1% hypochlorite solution

 

Meter Storage

  • Store on base unit
  • Purple box – if stored in purple box ensure meter docked after patient testing and at least once every 24 hours to transmit results and download updates.
  • Do not drop
  • Do not immerse in liquid

Do not expose to excessive heat eg phototherapy light, bed warmer, direct sunlight
