Review previous microbiology results: If resistant target organisms previously isolated (e.g. MRSA, ESBL, known resistance to empirical antibiotic choices), discuss antibiotic choice with Paediatric and/or Microbiology Consultant.

IV piperacillin/tazobactam andIV gentamicin
Suspected staphylococcal infection (e.g. line-related sepsis, skin/soft tissue infection) OR known MRSA positive: addIV vancomycin / IV teicoplanin

IV vancomycin andIV gentamicin
TOTAL duration
With clinical improvement, and on advice from Paediatric Consultant and/or Microbiology Consultant.
Comments
If haematology/oncology patient, discuss with appropriate specialist and seek local microbiology advice.
